This clinic provides quality care to the senior population through an interdisciplinary approach of outreach to the community and coordinates the planning, organizing, directing and evaluating of the care of senior clients admitted to the clinic for interdisciplinary assessment. The goal is to enable the senior client to remain as independent as possible and to be able to cope with the demands of community living.
The senior who would benefit from an interdisciplinary assessment is based on deficits identified in ADL, IADL, cognitive and/or support assessment.

Cardiac Assessment Clinics: Healing Hearts |
| Description: |
An eight-week program based on the New Brunswick Heart and Stroke Foundation guidelines, as well as the Canadian Association of Cardiac Rehabilitation recommendations. People will learn about heart disease, risk factors, nutrition, exercise, medications, handling emotions, stress management and how to make lifestyle changes. The nurse or physiotherapist will provide an individualized, home-based, exercise prescription to each participant. Each participant receives a log book to record his or her exercise habits. Each week the nurse or physiotherapist will review the logbook and make recommendations based on the participant’s progress. |

Cardiac Assessment Clinics: Pacemaker Clinic |
| Description: |
This clinic provides follow-up care for patients with a permanent pacemaker. All pacemakers require an initial check up after a new device has been implanted or an old one has been changed. These check ups are held between six and 12 weeks following the implant, and then every six or 12 months depending on the type and status of the pacemaker. |

| Description: |
Follow-up clinic for post cardiac surgery patients from New Brunswick and Prince Edward Island. This clinic uses the telehealth communication system to view patients. A camera is positioned in the exam room in Moncton to view the patient and nurse. This camera is connected to the Saint John Regional Hospital to allow the cardiologist from Saint John to see the room in Moncton. The cardiologist can then examine the patient and ask any questions. |

Diabetes Education Centre
The interdisciplinary health care team at the Diabetes Education Centre establishes and sustains a communication network between the patient and the necessary health care and community systems. The team is made up of physicians, nurses, dietitians and others.
Prediabetes Class
These classes, given by a dietitian and an exercise specialist from the YMCA provide people with a prediabetes explanation of how exercise and diet can help reduce blood sugar levels. Pulmonary Hypertension (PH) is the development of high blood pressure in the lungs and is a rare disorder affecting one in one million people each year. It is a progressive condition leading to decreased exercise tolerance and functional level. Pulmonary Hypertension cannot be cured but can be controlled. The Pulmonary Hypertension Clinic team includes a rheumatologist, lung specialists, PH nurse coordinator, respiratory technician and, if required, access to a dietitian and social work. Is my cast too loose or too tight?
If you think your cast is too tight, you have increasing pain, and/or your fingers or toes appear blue or feel cold, go to the Emergency department to have the cast examined by a physician. If your cast is loose or broken, call your physician’s office. If you are unable to contact your physician, go to the Emergency department.
Can I shower with a cast on?
The cast is made of plaster of paris or fibreglass. If it gets wet, it will fall apart. Before showering, enclose the entire cast in two plastic bags, fastening them securely so no water can get inside. Remove the plastic covering as soon as possible following the shower. |

The Maternal Fetal Care Unit deals with high-risk pregnancies relating to maternal and fetal complications of pregnancy as well as prenatal diagnosis. |

Neonatal Follow-Up Clinic |
| Description: |
This clinic provides follow-up care for premature babies (born at 32 weeks gestation and younger) that were born on the unit at The Moncton Hospital. Following discharge from the hospital, premature babies come in for their first visit at three months corrected age. The follow-up clinic includes a check up by a neonatologist, physiotherapist, occupational therapist, psychologist and speech-language pathologist and audiologist. |

Pediatrics Clinic |
| Description: |
This clinic provides services to ambulatory pediatric patients that cannot be provided in the physician’s office but do not require hospital admission. For example: oncology services like chemo administration, blood product transfusion and care of central lines; blood product transfusions for other patients; other types of infusions needed on a weekly/monthly basis; immunizations of high risk patients; allergy challenges for milk, eggs and peanut butter; difficult phlebotomy; phlebotomy procedure; sedation and monitoring for pediatric patients having radiology procedures; catheter insertion for VCUGs; insertion of long-term NG feeding tubes; diabetic education and follow-up; etc. |

Prenatal Health & Education Clinic |
| Description: |
The goal of the Women’s and Children’s Health Program is for the nurse at the Prenatal Health and Education Clinic to speak with all expectant mothers delivering at The Moncton Hospital. The clinic includes pertinent up-to-date information regarding policies and procedures in infant and maternal care and a tour of the Maternity Unit, including the birthing room. Any concerns the patient may have can also be addressed at this time.
Information regarding prenatal classes will be provided at the assessment. Classes include: Nutrition; Pregnancy; Labour & Delivery; Breastfeeding; Newborn & Mother Care; Infant/Child Development Class; Introduction to Solids to Baby. |

RSV Clinic |
| Description: |
This clinic provides protection against serious lower respiratory tract disease caused by respiratory syncytial virus (RSV) in infants and pediatric patients who are at risk for complications of RSV disease. |

| Description: |
The ambulatory care dietitians offer individual special needs nutrition counselling as well as lipid management and healthy living classes. |
